USAV / IREVA Registration 2011/2012 Season
All Registration Forms have been revised to reflect the
changes in the 2011-2012 registration requirements, membership options, and fee structures:
- Use of the new forms is required for registrations to be processed.
- Full-time college student fee has been reduced - see adult registration page for more details.
- A No Cost-8 & under membership has been added
- ALL Club Directors, Team Reps, Club Admins will be required to attend a Mandatory Annual Administrative Meeting in order to receive their club admin access.
- ALL Tournament Directors will be required to attend a Mandatory Annual Administrative Meeting in order to be granted sanctioning for their tournament/s.
REMINDERS:
- The Assistant Registrar, Susanna Ashline, will handle all registrations – boys, girls and adults (including Officials).
- Laurie Muollo is the Chief Registrar who over-sees the entire registration process, handles special situations sent to her by Susanna or BODs, and assists with problem solving throughout.
- Adult players who will also be associated with a Junior Club at some point in the season (as a coach, chaperone, etc.) MUST register with the Junior Club initially and indicate the desire for association with the Adult club to the Assistant Registrar in order to be placed on both rosters.
- Regular (full season) Membership application fees are $55, with an additional $5 fee for any paper applications.

- Registration for short term tryouts, camps and clinics (no more than 14 consecutive days for all sessions) are available for Junior members at the same $10 rate, and can also be upgraded should they decide to want a full season membership thereafter
- Select the “one-event / multi-day” option in WEBPOINT.
- Upgrades for one-day and multi-day memberships are handled via paper applications only in order to standardize processes and eliminate confusion and uncertainty caused by mail delays for delivery of applications and fees.
- Electronic signatures for:
- Background Checks are now included in WEBPOINT; signed forms only need to be submitted to IREVA if a paper application is being used in lieu of the on-line membership application and electronic signature. IREVA pays for all background checks from membership fees.
- Code of Ethics - USAV has created a standardized Junior Club Personnel Code of Ethics.
- The JCPCOE is required for all adults associated with a Junior club (not just coaches)
- Signed forms no longer need to be submitted to IREVA unless a paper application is being used in lieu of the on-line application and electronic signature.
- The IREVA’s policy on G-rated club and team names continues to be in place and enforced.

All Players / Members
- Obtain instructions from your Club Director or Team Rep on their detailed procedures for handling tryouts, registrations, and fees. There are numerous differences among the clubs with respect to how they meet IREVA’s requirements.
- All checks for club membership fees should generally be made out to and processed through the applicant’s club (with limited exceptions).
- Credit card payments will only be accepted with online applications for full memberships.
- During the on-line membership application process, please select the club with which you are joining.
- If you are trying out and acceptance to club is not assured, or you do not know your club, or may be joining a new club, select “Undecided”. Once you decide on a club, that Club Director will send in your name with the Team Application and you will be assigned to the correct club by the Assistant Registrar.
- Receipt of on-line applications will be confirmed by email from USAV;
- Actual processing of the applications for official membership does not occur until payment is received, membership type is approved, club paperwork is on file, and the Assistant Registrar hits the submit button. An additional confirmation email from USAV will be received at that point; if you don’t receive these emails, your application processes have not been completed!
- Junior Players - Apply for membership on-line / select JO club / parent on-line signature required / give copy of application confirmation*, medical form and any necessary fees to Club Director
- Non-playing Adults - JO Club Directors, Coach, Team Reps, Chaperone, Etc. - Apply for membership on-line / select JO club / Background Check required biennially / Code of Ethics required / give copy of application confirmation* and any necessary fees to Club Director.
- Adult Players also affliated with a JO club- Boys and Girls JO Club Directors, Coach, Team Reps, Chaperone, Etc. - Apply for membership on-line / select JO club / Background Check required biennially / Code of Ethics required / give copy of application confirmation* and fees to JO Club Director / also give copy of application confirmation* to adult club director / registrar will associate player with adult team
- Adult Player Only – Apply for membership on-line / select adult club / Give copy of application confirmation* and fees to Club Director
- Officials – Apply for membership on-line / select “Referee” club and affiliated with JO / Background Check required biennially / notify Mike McNeil (913 Wallace St. Endicott, NY 13760 – email: referee@ireva.org – phone: (607) 768-6502) of application confirmation / send fees to Assistant Registrar / IREVA pays background check fee for referees
